Job summary

The TemPositions Group is seeking a qualified Registered Nurse (BSN minimum) to assess patients, develop and intervene in health care plans, and supervise Licensed Practical Nurses on shift in a healthcare facility.

The role emphasizes advocacy, coordination with physicians, documentation, and collaboration across care teams from admission to discharge. CPR certification and strong communication are required, with at least two years of disability care experience.

Qualifications

• NYS Licensed Registered Nurse (BSN minimum).
• Minimum 2 years experience working with persons with disabilities.
• Extensive knowledge of pertinent health resources.
• Excellent oral and written communication skills.
• CPR Certification.
• Complies with applicable laws, rules, regulations, policies and procedures.

Responsibilities

• Assesses each person receiving services to determine needs and health care outcomes.
• Develops and updates health care plans and provides direct medical/nursing services.
• Oversees implementation of care plans and trains staff.
• Evaluates plan effectiveness and communicates outcomes.
• Refer to other health providers and schedule follow-ups.
• Ensures proper documentation of care and progress.
• Identifies need for conferences and consults with Nurse Supervisor.
• Maintains ongoing communication with physicians, supervisors and clinicians.
• Collaborates with care team from admission through discharge.
• May accompany individuals to clinics or outings.
• Develops strategies to prevent hospital admissions and deteriorations.
• Reviews clinic visit summaries and implements doctor orders.
• Updates patient profiles with care providers for appointments.
• Conducts regular medication administration observations and documentation.
• Oversees medication procedures and accountability with staff.
• Provides direct oversight to LPNs on shift and reports issues.
• In absence, supervises Direct Care staff and Day Program Nurse.
