Job summary

TemPositions Group of Companies is seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will coordinate services for the people we support, providing comprehensive patient care and ensuring well-being through managed care, patient assessment, and medical terminology expertise.

Qualifications

• Minimum of 1 year of experience as a Registered Nurse in a clinical setting.
• Experience in managed care or case management is highly preferred.
• Strong knowledge of medical terminology, physiology, and medical records management.
• Ability to assess patient needs and develop comprehensive care plans.
• Experience working with diverse patient populations, including pediatrics and behavioral health.
• Familiarity with emergency room protocols and procedures.

Responsibilities

• Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines.
• Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records.
• Conduct health assessments and respond to medical emergencies as needed.
• Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members.
• Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP).
• Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols.
• Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies.
• Provides supervision and training for Licensed Practical Nurses and/or less experienced.
• Provides immediate first aid with referral to outside emergency services or private physician.
