Job summary

TemPositions Group of Companies in New York seeks a dedicated Registered Nurse Case Manager to join our Social Service Health Care Team. You will coordinate services, assess patient needs, and develop person-centered care plans for individuals with developmental disabilities within OPWDD guidelines.

The role requires 1+ year of RN experience, strong knowledge of medical terminology, and ability to collaborate with DSPs, medical providers, therapists, and families.

Qualifications

• Active RN license in the state of New York.
• Minimum 1 year of RN experience in a clinical setting.
• Experience in managed care or case management is preferred.
• Strong knowledge of medical terminology, physiology, and medical records management.

Responsibilities

• Provide direct nursing care and clinical oversight for individuals within OPWDD guidelines.
• Administer prescribed medications and treatments and maintain medical records.
• Conduct health assessments and respond to medical emergencies.
• Collaborate with DSPs, medical providers, therapists and families.
• Develop and update individualized nursing care plans and ISP.
• Educate staff on health-related issues and infection control.
