Job summary

Tuba City Regional Health Care Corporation seeks a Nurse Case Manager to coordinate care for high-risk patients at TCRHCC and Echo Cliffs Health Center. You will assess needs, plan services, and monitor progress with an interdisciplinary team to ensure safe discharges and post-hospital referrals.

Responsibilities include evaluating ED medical necessity, arranging outpatient plans, and supporting patients with complex social and health barriers, including mental health navigation as needed.

Qualifications

• Active RN license in the United States or territories.
• 3 years of clinical nursing experience; 1 year in outpatient/inpatient.
• Knowledge of case management, contract health, and utilization review processes.

Responsibilities

• Provide comprehensive, client-centered case management for high-risk patients.
• Coordinate care transitions from admission to discharge and post-acute referrals.
• Collaborate with interdisciplinary teams to maintain safe, efficient hospitalizations and care plans.
