Qualifications

• Minimum 1 year of RN case management experience (CM II).

Responsibilities

• Coordinates necessary post-acute care services and ensures appropriate levels of care.
• Gathers data to verify admission criteria and stay requirements.

Education

Associate Degree in Nursing (ADN)

Current CA Registered Nurse (RN) license

Current Basic Life Support (BLS) certification

One year of case management experience (RN CM II)
