Job summary

Advocate Medical Center seeks a Case Manager to apply the nursing process to develop, implement, and evaluate outcomes for defined patient populations.

You will collaborate with physicians, nurses, and staff to advocate for continuity of care, effective communication, and appropriate services, requiring RN licensure and a minimum of five years' experience; BSN preferred, MSN encouraged.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses.
• Current RN license to practice in the applicable state.
• Minimum of five years of experience in care of defined patient populations.
• Participation in case management and utilization review encouraged.
• Case management certification encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
• Educate, consult, and assist in managing identified patients to achieve high quality, cost-effective outcomes.
• Collaborate with a patient care team including physicians, nurses, and allied disciplines to ensure continuity of care.
