Job summary

Your Health is seeking a Registered Nurse to support patients across the Charleston area as a Visiting Nurse (Nurse Case Manager). The role includes home visits, facility visits, and initiating telemedicine, with daily travel between offices and sites.

The position is full-time, 8-hour shifts, Monday–Friday (8am–5pm). We deliver comprehensive primary care and support across homes, clinics, and care facilities, with competitive compensation, 401K matching, and generous PTO.

Qualifications

• Must be a licensed nurse in good standing.
• Minimum 3 years of clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams.

Responsibilities

• Facilitate receiving medical records from patients’ providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management when eligible.
• Initiate and monitor Care Management Plans of Care.
• Capture diagnoses with specificity and address gaps.
• Complete AWVs and cognitive impairment screenings.
• Perform SDoH assessments and ACPs as needed.
• Coordinate home health, hospice, or consults as appropriate.
• Assess RPM devices and tools to improve quality of life.
• Document Care Management activities and keep plans up to date.
• On-call may be required.
