Job summary

Ascension Sacred Heart Pensacola is seeking a dedicated Case Management Registered Nurse to oversee care from admission to discharge, coordinating with physicians, nurses and social services to align on patient goals.

You will design care strategies across the continuum, monitor progress, and ensure medical records accurately reflect diagnoses and treatment plans. Florida RN licensure and prior case management experience are required; BLS is preferred.

Qualifications

• Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Responsibilities

• Lead the development and execution of a comprehensive care plan from admission to discharge, ensuring all healthcare providers are aligned on the patient’s goals.
• Design care strategies across the continuum that integrate the specific preferences, values, and long‑term needs of the patient and their family.
• Proactively monitor patient progress through clinical assessments to identify and eliminate obstacles to care or resource gaps.
• Serve as a technical resource for medical record accuracy, ensuring that all services and post‑discharge options align with the patient's diagnosis and treatment plan.
