Qualifications

Responsibilities

• Directs, coordinates, and provides case management to patients in caseload.
• Prevents delayed discharges of observation patients.
• Participates in preadmission evaluation screening to ensure a safe discharge plan.
• Completes case management assessment for timely transition plan.
• Interacts with multidisciplinary healthcare team to optimize resource use.
• Maintains professional commitment to department goals.
• Adapts to floor and work schedule as needed.
• May be required to remain on campus during severe weather.
