Job summary

University Health Careers is seeking a nurse case manager to coordinate the treatment plan for patients during the acute episode of care, collaborating with the patient and interdisciplinary team.

You will proactively facilitate timely delivery of services, evaluate the effectiveness of interventions, and track barriers in the plan of care. As the patient advocate, you identify and communicate health care needs to the team.

Qualifications

• Bachelor’s degree in Nursing; highly preferred.
• Three to five years nursing experience required.
• Experience in case management, utilization review or hospital quality preferred.

Responsibilities

• Coordinate treatment plan for patient within acute episode.
• Facilitate timely delivery of services and evaluate interventions.
