Qualifications

• Work experience in case management, utilization review or hospital quality is preferred.

Responsibilities

• Coordinate treatment/pl an for a patient within the acute episode of care in collaboration with the patient and interdisciplinary team.
• Facilitate interventions to assure timely delivery of services and evaluate effectiveness of interventions.
• Track variances/barriers in the plan of care and advocate for patient needs.
