Job summary

Qualifications

• Work experience in case management, utilization review or hospital quality is preferred.

Responsibilities

• Coordinate care for patients during the acute episode of care, in collaboration with the patient and interdisciplinary team.
• Facilitate timely delivery of services and evaluate effectiveness of interventions.
• Act as patient advocate to identify and communicate health care needs and track plan variances/barriers.
