Highmark Health’s Allegheny Health Network is seeking an experienced RN Case Manager to develop, implement, and evaluate care plans for patients and families across the health continuum in collaboration with clinicians, payers, and post‑acute partners.

The role emphasizes advocacy, education, and efficient resource use, with a focus on reducing readmissions, length of stay, and ED utilization while promoting patient satisfaction.

Qualifications

• Bachelor’s degree in nursing or equivalent credentials is required.
• 3 years in a clinical nursing role are required.
• Current PA RN licensure or multi-state licensure via eNLC.
• Critical thinking and problem solving are essential.
• Flexibility and adaptability to change are important.
• Strong communication and collaboration with diverse teams.

Responsabilités

• Contributes to or completes comprehensive assessment of patients and families, including social determinants and barriers.
• Identifies barriers and analyzes situations for safe transition and care coordination.
• Coordinates clinical care with patient, family, provider, and interdisciplinary team.
• Advocates for patients through effective communication with providers and post-acute partners.
• Educates patients and families on care, documentation, utilization, and transition planning.
• Pursues professional development through certifications, mentoring, and committee work.
• Adheres to hospital policies and clinical guidelines.
