Job summary

UW Medical Center-Montlake is seeking a Nurse Care Coordinator for per diem coverage to enhance inpatient flow and ensure safe, timely discharges. The role collaborates with the care team to align plans of care and maximize patient outcomes.

The position requires an MSN or related field, with at least three years of nursing experience and preferred case management expertise. WA RN licensure and BLS are required. Per diem shifts contribute to a flexible schedule on the Montlake campus.

Qualifications

Master's degree in Nursing or related field; experience may substitute for education.
Three years of professional nursing experience; case management preferred.
Strong English communication skills, both written and verbal.
Interpersonal leadership skills and clinical expertise.
Ability to access and document in electronic medical records; WA RN license with BLS.

Responsibilities

Identify and monitor high-need patients to ensure timely consults, tests, and coordination.
Lead medical team and nursing huddles; remove discharge barriers.
Assist with ordering complex equipment and safe transitions of care.
Develop and monitor outcomes for discharge planning and staff education.
