Job summary

Compass Health, Inc. in Arnold, MO is seeking a Nurse Care Manager for the Behavioral Health Healthcare Home.

You will work with a multidisciplinary team to deliver integrated physical and behavioral health care for clients with serious mental illness and co-occurring conditions. You'll manage a caseload, perform health screenings, set physical health goals, coordinate preventive care, and provide education to clients and teams.

Qualifications

• High School/GED is required.
• Practical Nursing or Associate Degree in Nursing is required.
• Bachelor’s degree in nursing is preferred.
• Experience in behavioral health or integrated care settings is preferred.
• A valid driver’s license and automobile coverage required.
• LPN or RN licensure/certification is required.

Responsibilities

• Manage a caseload of up to 200 clients in the Healthcare Home program.
• Complete health screenings to assess risk and care gaps.
• Develop physical health goals and promote healthy lifestyle habits.
• Coordinate preventive care services including PCP and referrals.
• Perform metabolic syndrome screenings and med reconciliations post-hospitalization.
• Educate clients and team members on chronic conditions and self-management.
• Collaborate with providers and care teams for integrated, person-centered care.
• Attend team huddles, staff meetings, and program consults.
• Provide in-person services at clinics and occasional home visits.
• Maintain compliance with DMH regulations and Compass Health standards.

Support Whole-Person Wellness as a Nurse Care Manager – Behavioral Health Healthcare Home
Schedule: M-F 8-5

As a Nurse Care Manager in our Behavioral Health Healthcare Home program, you’ll be a key part of a multidisciplinary team providing holistic care to clients with serious mental illness and co-occurring conditions. You'll deliver targeted interventions that help close health gaps and empower individuals to lead healthier, more independent lives.

Key Responsibilities

• Manage a caseload of up to 200 clients enrolled in the Healthcare Home program
• Complete health screenings to assess risk and identify gaps in care
• Develop physical health goals and promote healthy lifestyle habits (nutrition, exercise, tobacco cessation)
• Coordinate preventive care services including PCP and specialist referrals
• Perform metabolic syndrome screenings and post-hospitalization medication reconciliations
• Educate clients and team members on chronic health conditions, prevention and self-management
• Collaborate with providers, care coordinators, and support teams to ensure integrated, person-centered care
• Attend team huddles, staff meetings, and consults across behavioral health programs (CPRC, SUD, ACT, TAY, STEPS)
• Provide in-person services at assigned clinics and occasional home visits as needed
• Maintain compliance with DMH regulations and Compass Health’s quality standards

Requirements, Skills, Knowledge and Expertise

High School/GED required

• Practical Nursing or Associate of Nursing degree required
• Bachelor’s degree in nursing preferred

Work Experience/Training/Additional Requirements:

• Experience in behavioral health or integrated care settings preferred
• A valid driver's license and agency established minimum automobile coverage required

Licensure/Certification:

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) required
