Job summary

Aroostook Mental Health Services, Inc. is seeking a compassionate Nurse Care Manager to join our integrated outpatient team. This full-time exempt role bridges physical and behavioral healthcare and collaborates with behavioral health professionals, primary care providers, hospitals, and community partners.

The ideal candidate will have a BSN, Maine RN licensure, and 2+ years in behavioral health or integrated care. Travel across the service area is required to support coordinated care.

Qualifications

  • Bachelor of Science in Nursing is required.
  • Active RN licensure in the state of Maine is required.
  • Two (2) years nursing experience preferred, especially in behavioral health or integrated care settings.

Responsibilities

  • Coordinate medical and behavioral healthcare services with multidisciplinary teams.
  • Support individuals transitioning from hospitals, crisis services, residential programs, and other settings.
  • Conduct ongoing assessments of medical and psychiatric needs via chart reviews, client contacts, and provider collaboration.
  • Coordinate care with primary care providers, specialists, pharmacies, hospitals, and community resources.
  • Review medical records, lab results, medications, and treatment plans to promote continuity of care.
  • Educate clients, families, caregivers, and staff on wellness, medications, symptom management, and preventive care.
  • Develop and facilitate health and wellness groups.
  • Participate in quality improvement initiatives and agency-wide integrated care efforts.
  • Provide nursing consultation and support to residential and crisis programs via nursing on-call rotation.

In this rewarding role, you'll work alongside behavioral health professionals, primary care providers, hospitals, residential programs, and community partners to ensure individuals receive coordinated, high-quality care that supports their overall health and wellness.

What You'll Do

As a Nurse Care Manager, you will:

  • Collaborate with multidisciplinary teams to coordinate medical and behavioral healthcare services.

Schedule
  • Full-time, exempt position
  • Monday through Friday, generally 8:00 a.m. – 5:00 p.m.
  • Participation in the agency's nursing on-call rotation required.
  • Travel throughout the service area as needed.
Requirements
  • Bachelor of Science in Nursing is required.
  • Active licensure as a Registered Nurse (RN) in the state of Maine is required.
  • Minimum of two (2) years of nursing experience preferred, with experience in behavioral health, community mental health, substance use treatment, primary care, care management, or integrated healthcare settings strongly preferred.
  • Valid driver’s license, reliable transportation, and proof of automobile insurance required, with the ability to travel within the service area as needed.
