Qualifications

• Master's degree in Nursing or related field required; applicable experience may substitute for education.
• Three years of professional nursing experience; case management experience strongly preferred.
• Able to communicate effectively in English, both verbally and in writing.
• Effective interpersonal and leadership skills with clinical expertise.
• Ability to access information and document in electronic medical records.

Responsibilities

• Identify and monitor high-need patients to ensure timely consults, procedures and tests; facilitate coordination across providers.
• Lead medical team and nursing huddles to remove barriers to discharge due to medical or system obstacles.
• Assist with ordering complex medical equipment and ensure safe transitions of care.
• Develop and monitor outcome measures related to care coordination and staff/patient satisfaction with discharge planning.
