Job summary

UW Medical Center - Northwest is seeking a Nurse Care Coordinator to join the Care Management team. The role focuses on achieving coordinated patient flow through inpatient stays and ensuring safe, timely discharges.

You will lead interdisciplinary team huddles, address barriers to discharge, and support equipment ordering and care transitions within the UWMC Northwest system. Qualified candidates hold a BSN, have at least three years of nursing experience (care management preferred), and

Qualifications

• Bachelor's degree in Nursing required; master's preferred or relevant experience may substitute for education.
• Three years of professional nursing experience; care management experience strongly preferred.
• Ability to communicate effectively in English, both verbal and written; strong interpersonal skills and clinical expertise.

Responsibilities

• Identify and monitor high-need patients to ensure timely consults, procedures, and tests; maintain ongoing team communication and coordinate care.
• Lead medical team huddles; remove barriers to discharge due to medical complications or system obstacles.
• Assist with ordering complex medical equipment and ensure safe transitions of care; monitor discharge planning outcomes.
• Provide ongoing consultation and staff education to improve patient flow through UWMC systems.
