Job summary

Advocate Aurora Health Care is seeking a qualified registered nurse with strong case management and discharge planning skills. The role emphasizes interdisciplinary collaboration, CMS compliance, and utilization management to ensure timely and efficient patient transitions.

The position requires a Bachelor’s Degree in Nursing and at least 2 years of clinical nursing experience. The incumbent will work with the care team to coordinate patient needs across the hospital stay and after discharge.

Responsibilities

• Conducts complete assessments, establishes appropriate plans, and initiates interventions within desired timeframes.
• Collaborates with patient, family and team to achieve care goals and discharge planning objectives.
• Provides case management across health care, finances, housing, and illness adjustment.
• Documents discharge planning and utilization review activities per department standards.
• Communicates with the healthcare team; partners with Social Work and support staff for safe care plans.
• Maintains CMS regulatory notices and coordinates with payers/external review when needed.
• Develops relationships with community agencies to support patient transitions.
• Educates staff regarding admission status, care coordination, and regulatory requirements.
• Demonstrates knowledge of growth, development, and age-specific care needs.
