Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with laws, standards, regulations, and Sunrise policies to ensure the highest quality of care.

Responsibilities & Qualifications
Responsibilities:
  • Support Sunrise's Mission, Principles of Service, and Core Values, emphasizing the sacred value of human life.
  • Administer medications and treatments, ensuring safety and proper documentation.
  • Review, read, notate, and initialize the Daily Log for resident information.
  • Receive medication updates from the RCD or Wellness Nurse.
  • Administer, assist with, and observe medications using the medication administration record and the Six Rights of Medication Pass.
  • Ensure medications are passed on time, document refusals or missed doses, and maintain confidentiality.
  • Report resident concerns, restock medication carts, and assist in medication checks.
  • Maintain cleanliness and organization of medication areas.
  • Assist residents in maintaining independence and dignity, practicing good standard care precautions.
  • Assess residents for 'as needed' medications and report accordingly.
  • Count narcotics with another MCM or LCM each shift.
  • Participate in resident care planning, updates, and transcribing orders.
  • Ensure safety and compliance with OSHA, infection control, and other safety regulations.
  • Report accidents, unsafe conditions, and occupational exposures immediately.
  • Participate actively in team meetings, training, and engagement programs.
  • May serve as shift supervisor or supervise other medication care managers.
Qualifications:
  • Licensed nurse credential (LPN, LVN, or equivalent).
  • Maintain required certifications, including CPR and First Aid.
  • Be at least 18 years old.
  • Previous experience with seniors preferred.
  • Strong desire to serve and care for seniors.
  • Ability to make decisions, react calmly, and handle multiple priorities.
  • Effective communication skills and good judgment.
