THIS POSITION IS 20 HOURS PER WEEK. IT WILL BE FIVE 4-HOUR SHIFTS PER WEEK.

• ROTATING/DAY SHIFT HOURS:
• MONDAY-THURSDAY 5:00PM-9:00PM
• FRIDAY 4:00PM-8:00PM
• SATURDAY/SUNDAY 8:00AM-12:00PM or 4:00PM-8:00PM

The Triage RN, Access role reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory standards.

The Triage RN, Access provides clinically expert virtual triage service, serving as a liaison between the patient and their physician by assessing presenting problems, utilizing evidence-based triage protocols to identify acuity, and developing appropriate care advice and next-step recommendations.

Responsibilities

• Demonstrate excellent customer service and communication skills, functioning as part of a team that puts Patients First.
• Establish positive first impressions with patients, advocating for them.
• Triages all incoming calls and messages, prioritizes and resolves issues, involving providers as needed, and completes communication cycles.
• Develops and updates individualized care plans based on patient needs.
• Asks key questions to understand clinical conditions and adjusts approach accordingly.
• Documents all communications in the electronic medical record.
• Schedules patient visits with providers as appropriate.
• Follows triage protocols and assists in scheduling appointments.
• Participates in orientation of new staff and process improvement projects.
• Engages in departmental quality improvement activities.
• Uses effective service recovery skills to resolve problems or service breakdowns.

Qualifications

Required:

• Graduate of an accredited RN program.
• At least 2 years of ambulatory care experience.
• Registered Nurse License issued by the State of Illinois.
• Current AHA BLS Provider certification.

Preferred:

• BSN.
• Specialty certification upon eligibility.
