Job summary

Hackensack Meridian Health seeks an Advanced Practice Nurse (APN) to lead patient-centered care within a coordinated model. The APN will collaborate with physicians and hospital teams to assess, plan, and implement care for individuals with health and safety needs.

Requirements include an NP degree (MSN/DSN/PhD), active NJ NP license, NPI, prescriptive authority, and strong communication skills. Weekend and holiday coverage may be required; on-site practice with ongoing professional development

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting per NJ regulations.
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration per NJ state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• BLS/ACLS certification as applicable.

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Hospice and Palliative experience preferred.

Excellent written and verbal communication skills. 9. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License. 2. Advance Practice Nurse License. 3. BLS AHA Basic Health Care Life Support HCP Certification. 4.
