Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an experienced Nurse Practitioner or Physician Assistant to provide autonomous primary and episodic care in skilled nursing facilities.

You will manage your own panel and build long-term relationships with patients and families. The position features a flexible schedule, 6-hour shifts, home charting, and productivity-based compensation with high earning potential.

Qualifications

• Board certification required (AGNP/FNP for NP; PA-C for PA)
• Active, unrestricted NP/PA license in the operating state
• Master's degree in Nursing (NP) or PA program credentialed
• Skilled nursing or long-term care experience strongly preferred
• Strong independent clinical judgment in fast-paced settings

Responsibilities

• See 16–20 patients per day in a mix of long-term and acute issues
• Conduct annual wellness visits and H&Ps; manage chronic conditions
• Respond to acute changes; review meds and orders
• Coordinate with physicians, specialists, and facility staff; join care conferences
• Maintain communication with families to keep care seamless
