Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage members. The role emphasizes education, gap closure, and collaboration with primary care providers in a field-based setting.

You will conduct comprehensive assessments, document findings, and support care planning while working a flexible schedule within a 7-day week. No on-call, weekends, or holidays required.

Qualifications

• Active ANCC or AANP certification for NP.
• NCCPA certification and state licensure for PA.
• Active, unrestricted licensure in the state you will practice in.
• Driver's license with reliable transportation for home visits.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage members.
• Document diagnoses and care plans for care management.
• Communicate findings to PCPs to address gaps in care.
• Educate members on disease processes, medications and compliance.
• Identify urgent/emergent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
