Qualifications

• 1+ years of clinical experience preferred.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members.
• Communicate findings to members’ PCPs to address gaps in care.
• Educate members on disease processes and medication use.

Skills
