Zusammenfassung

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members and other plan participants. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible schedules, no on-call or weekend requirements, and a chance to advance within a supportive Interdisciplinary team.

Qualifikationen

• Active, unrestricted clinical license in the practicing state
• Active drivers license and reliable transportation for home visits
• 1+ years of clinical experience in family, geriatric, or home health preferred

Aufgaben

• Conduct comprehensive in-home assessments including medical history, vital signs, medication reconciliation and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify diagnoses for care management and treatment planning
• Communicate findings to PCPs to address care gaps
• Educate members on disease processes, medications and compliance
• Address social determinants of health and provide referrals as needed
