Job summary

Optum HouseCalls is seeking an Advanced Practice Clinician to perform in-home health assessments for Medicare Advantage and other members. You will work field-based with no on-call, weekends, or holidays, and schedules run 8am–8pm, 20–40 hours weekly.

Role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes. Requires NP/PA credentials, current licensure, and driver’s license for home visits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in practicing state
• Active and unrestricted driver's license with reliable transportation
• Ability to navigate varied home environments and perform assessments
• If independent practice is allowed, obtain licensing board approval within required timelines

Responsibilities

• Conduct comprehensive in-home assessments, including past medical history, med reconciliation, vital signs and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications and adherence
• Address social determinants of health and provide referrals as needed
