Qualifications

• Active NP or PA certification and state license.
• Experience in home health or geriatric care is preferred.
• Excellent communication with patients and PCPs.

Responsibilities

• Conduct in-home health assessments for Medicare members.
• Document diagnoses and care plans for management.
• Educate patients about diseases, meds and adherence.
• Coordinate care with primary care providers and referrals.
