Qualifications

• ANCC/AANP Family, Adult, Geriatric, or Adult-Gerontology Primary Care certification.
• NCCPA certification and state licensure for PAs.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning and communicate findings to PCPs.
• Educate members on disease processes, medications, and adherence; address social determinants of health.
