Optum's Senior Community Care team seeks an Advanced Practice Clinician to provide longitudinal, high-quality care to medically complex patients in community-based residential settings across Marion County. Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions with diagnostics and interventions.
• Develop and manage complex treatment plans.
• Lead goals-of-care and advance care conversations.
• Collaborate with interdisciplinary care teams and caregivers.
• Provide patient and caregiver education.
• Ensure complete clinical documentation.
