A healthcare organization is seeking an experienced Nurse Practitioner to provide in-home and virtual chronic care. This role emphasizes delivering high-quality care to low-income patients with a $10,000 sign-on bonus. Candidates should have a Nurse Practitioner license in Michigan and at least 2 years of primary care experience. The role offers a base salary of $120,000 to $145,000 plus benefits.

Qualifications

• Active DEA registration or eligibility; X-waiver for buprenorphine is a plus.
• Valid unrestricted driver’s license and access to an insured vehicle.

Responsabilités

• Provide patient-centered, holistic primary care via in-home and virtual visits.
• Develop and lead comprehensive care plans for patients.
• Collaborate with external providers for patient care.

Connaissances

Active Nurse Practitioner license in Michigan

AANC or AANP board certification

2+ years of clinical experience in primary care

Comfort with electronic medical records

Excellent communication skills

Formation

Certified in Basic Life Support for Healthcare Providers
