Job summary

Penn State Health is seeking a full-time Nurse Practitioner for our Post Acute Care Program in suburban Lancaster County, PA. You will provide patient-centered care in long-term care settings, collaborating with an experienced team and a Penn State Health physician to support optimal outcomes.

Qualifications

  • Master’s Degree in Nursing is required.
  • Two (2) years of RN work experience is required.
  • Current license to practice as a CRNP by the Pennsylvania Board of Nurse Examiners is required.
  • BLS certification is required.

Responsibilities

  • Provide patient-centered care for post-acute rehabilitation and long-term care patients.
  • Collaborate with Penn State Health board-certified physicians and interdisciplinary teams to optimize outcomes.
  • Support quality of life improvements and comprehensive care planning.
  • Maintain a Monday–Friday day schedule with shared call coverage.

JOB SUMMARY:

Penn State Health Post Acute Care Program is seeking a full-time Nurse Practitioner to join a dedicated and growing team committed to delivering exceptional care to patients receiving rehabilitation and long-term care services.
This position offers the opportunity to practice in one, and no more than two, high-quality long-term care communities in suburban Lancaster County, Pennsylvania, where you will play a vital role in providing comprehensive, patient-centered care. The Nurse Practitioner will work collaboratively with a Penn State Health Board-Certified Physician and an experienced interdisciplinary team to support optimal patient outcomes and enhance quality of life.

Schedule & Work-Life Balance

MINIMUM QUALIFICATIONS:
  • Master’s Degree in Nursing with clinical specialization required.
  • Two (2) years' work experience as an RN required.
  • Current licensed to practice as a CRNP by thePennsylvania Board of Nurse Examiners required.
  • BLS Certification required
PREFERRED QUALIFICATIONS:

Previous Internal Medicine Experience. Inpatient and/or LTAC-Rehab clinical experience. Experience working with the Geriatric population.
