Job summary

Penn State Health is seeking a full-time Nurse Practitioner for our Post Acute Care Program in suburban Lancaster County, PA. You will provide patient‑centered NP care in rehabilitation and long‑term care settings, collaborating with a board-certified physician and an experienced team to improve outcomes.

This role offers a Monday–Friday schedule with shared telephone call coverage, emphasizing autonomy, quality, and compassionate care within Penn State Health Lancaster Medical Center

Qualifications

  • Master’s Degree in Nursing required.
  • Two (2) years' work experience as an RN required.
  • Current licensed to practice as a CRNP by the Pennsylvania Board of Nurse Examiners required.
  • BLS Certification required.

Responsibilities

  • Provide patient‑centered NP care in rehabilitation and long‑term care settings.
  • Collaborate with a Penn State Health Board‑Certified Physician and an interdisciplinary team.
  • Support optimal patient outcomes and quality of life in post‑acute care.
  • Maintain a Monday–Friday schedule with shared telephone call coverage.

JOB SUMMARY:

Penn State Health Post Acute Care Program is seeking a full-time Nurse Practitioner to join a dedicated and growing team committed to delivering exceptional care to patients receiving rehabilitation and long-term care services.

This position offers the opportunity to practice in one, and no more than two, high-quality long-term care communities in suburban Lancaster County, Pennsylvania, where you will play a vital role in providing comprehensive, patient-centered care. The Nurse Practitioner will work collaboratively with a Penn State Health Board-Certified Physician and an experienced interdisciplinary team to support optimal patient outcomes and enhance quality of life.

MINIMUM QUALIFICATIONS:

  • Master’s Degree in Nursing with clinical specialization required.
  • Two (2) years' work experience as an RN required.
  • Current licensed to practice as a CRNP by thePennsylvania Board of Nurse Examiners required.
  • BLS Certification required

PREFERRED QUALIFICATIONS:

Previous Internal Medicine Experience. Inpatient and/or LTAC-Rehab clinical experience. Experience working with the Geriatric population.
