Penn State Health Post Acute Care Program is seeking a full-time Nurse Practitioner to join a dedicated and growing team committed to delivering exceptional care to patients receiving rehabilitation and long-term care services.
This position offers the opportunity to practice in one, and no more than two, high-quality long-term care communities in suburban Lancaster County, Pennsylvania, where you will play a vital role in providing comprehensive, patient-centered care. The Nurse Practitioner will work collaboratively with a Penn State Health Board-Certified Physician and an experienced interdisciplinary team to support optimal patient outcomes and enhance quality of life.

Previous Internal Medicine Experience. Inpatient and/or LTAC-Rehab clinical experience. Experience working with the Geriatric population.
