Job summary

Hearthside is seeking an RNAC/LPNAC Assessment Coordinator to join our skilled nursing facility team in State College, PA. You will oversee daily nursing operations, mentor staff, and coordinate resident assessments and the RAI process in compliance with federal, state, and facility regulations.

The role requires at least 2 years of clinical nursing experience, leadership experience, and PA RN or LPN licensure.

Qualifications

• Minimum 2 years of clinical nursing experience, preferably in long-term care or skilled nursing.
• Leadership or supervisory experience, preferably in long-term care or skilled nursing.
• RNAC or LNAC experience
• Registered Nurse (RN) license in Pennsylvania OR Licensed Practical Nurse
• Strong organizational skills and attention to detail
• Excellent communication and teamwork abilities

Responsibilities

• Oversee daily nursing operations and support/mentor nursing staff
• Conduct and coordinate resident assessments and the RAI process in compliance with federal, state, and facility regulations
• Maintain quality assurance programs and monitor QI/QM reports to address potential issues
• Serve as a clinical resource and provide guidance to the interdisciplinary team
• Participate in facility surveys and inspections
• Deliver initial and ongoing education related to resident assessment and care planning
