Transitions Healthcare North Huntingdon in North Huntingdon, PA is seeking a Nurse Assessment Coordinator (RNAC or LPNAC). This role collaborates with the MDS Coordinator to plan and complete MDS assessments, ensuring compliant care plans and accurate resident records.

You will support nursing staff, review MDS data, attend Medicare meetings, and educate the team on MDS processes while maintaining organized, behind-the-scenes operations that keep resident care on track.

Qualifications

• Current Pennsylvania LPN or RN license (required).
• CPR/AED certification (required).
• At least 1 year of nursing experience (required).
• Proficient in Microsoft Office Suite and Outlook.

Responsibilities

• Gather the data needed to complete the MDS.
• Help identify significant changes in residents' conditions.
• Monitor resident care plans for accuracy and completeness.
• Attend the facility’s Medicare meeting.

Transitions Healthcare North Huntingdon is hiring a Nurse Assessment Coordinator! This position is open to both RNs and LPNs.

As our Nurse Assessment Coordinator, you’ll work closely with our MDS Coordinator to plan and carry out MDS completion, following facility procedures along with state and federal regulations. Accurate assessments are what make sure every resident gets the right care plan and the support they need, so your attention to detail will have a real impact on their day-to-day lives.
