Job summary

Sweetwater Care in Fresno, CA is seeking an experienced LVN/LPN to provide nursing care within scope of practice and in accordance with standards and physician orders. You will lead nursing assistants and ensure adherence to policies, while admitting, transferring and discharging residents as needed.

The role requires a current California LVN/LPN license and active BLS/CPR certification. Experience with PointClickCare charting and prior nursing management is preferred, but not mandatory.

Qualifications

• Experience in long-term care settings is required.
• Proficient with PointClickCare charting.
• Must hold current LVN/LPN license in California.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience is preferred but not required.

Responsibilities

• Lead day-to-day nursing assistants in accordance with rules and guidelines.
• Ensure nursing staff comply with policies and procedures.
• Admit, transfer, and discharge residents as needed.
• Document charting and incident reports per procedures.
• Oversee narcotics counting, pharmacy orders, and restraint reduction.
• Monitor changes in patient conditions and communicate with the care team.
• Use the Quality Life Manual to identify and monitor patient programs.
• Notify departments of room changes, LOA, admissions, and discharges.
• Make rounds with physicians as needed.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary.
