Job summary

Sweetwater Care in California is seeking an LVN/LPN to provide compassionate nursing care within the scope of practice, following physician orders and center policies. You will lead day-to-day nursing staff, ensure compliance with regulations, and coordinate admissions, transfers, and discharges.

Strong charting discipline with attention to state guidelines is essential. Qualified candidates have long-term care experience, familiarity with PointClickCare, and a valid LVN/LPN license; BLS/CPR

Qualifications

• Long-term care experience required.
• Proficient in PointClickCare charting.
• Active LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is preferred but not required.

Responsibilities

• Lead daily nursing assistant activities in compliance with rules and guidelines.
• Ensure all nursing staff follow written policies and procedures.
• Admit, transfer, and discharge residents as needed.
• Maintain accurate charting and incident reporting.
• Manage narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions and notify appropriate departments.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient’s conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient’s room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary.
