Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice and ensure patient needs are met per standards and orders.

The role includes leading nursing assistants, adhering to policies, and coordinating admissions and discharges while maintaining proper charting and reporting practices. Rounds with physicians may be required; equal employment opportunity policies apply.

Qualifications

• Must have long term care experience.
• Facility uses Point Click Care charting; familiarity required.
• Current LVN/LPN license in practicing state.
• BLS/CPR certification current or willingness to obtain.

Responsibilities

• Lead day-to-day functions of nursing assistants.
• Ensure nursing staff comply with policies and procedures.
• Admit, transfer, and discharge residents as needed.
• Follow established charting and incident reporting procedures.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions (nutrition, infections, skin, weight, etc.).
• Utilize the Quality Life Manual to identify, place, and monitor patients.
• Document and notify departments of room changes, LOA, admits, or discharges.
• Make rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary.
