Job summary

Sweetwater Care in California is seeking an LPN/LVN nurse to provide bedside nursing within scope of practice, following physician orders and center policies.

You will lead nursing assistants, ensure policy compliance, and participate in rounds with physicians to monitor residents' conditions.

This role requires CA LVN/LPN license, long-term care experience, BLS/CPR certification, and willingness to engage in nursing management responsibilities.

Qualifications

• Requires long-term care experience.
• Experience with PCC charting.
• Active LVN/LPN license in CA.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Lead day-to-day nursing assistants in accordance with rules and guidelines.
• Ensure nursing staff comply with policies and procedures.
• Admit, transfer, and discharge residents as needed.
• Follow procedures for charting and incident reporting.
• Manage narcotics counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions and notify applicable staff.
• Round with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary.
