Job summary

Sweetwater Care is seeking a skilled licensed vocational nurse (LVN/LPN) in California to provide competent nursing care within the scope of practice, and in line with physician orders and center policies.

The role supports patient welfare through accurate documentation, adherence to standards, and collaboration with physicians. Strong charting, crisis management, and patient observation are essential to success.

Qualifications

• Long Term Care experience required.
• Nursing management experience preferred.
• Point Click Care charting experience desirable.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional issues, infections, skin conditions, weight loss or other changes.
• Uses the Quality Life Manual for patient placement and monitoring in appropriate programs.
• Documents and notifies relevant departments of patient room changes, LOA, admits or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
