Qualifications

• Current RN license in the state of employment.
• Minimum 1 year of nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• English communication skills required; Spanish preferred.

Responsibilities

• Make after-hours visits to patients in homes or facilities and provide compassionate care.
• Assess patient/family needs and provide appropriate nursing interventions.
• Deliver direct patient care as part of the IDG; coordinate with other professionals.
• Educate patients and families on disease, palliative care, safety, and care options.
• Document care per policy; communicate updates via SBAR.
• Attend patient deaths and support bereavement processes; ensure proper discharge planning.
• During emergencies, report to work as required to maintain coverage.
