Job summary

Responsibilities include taking vital signs, assisting with bathing, grooming, mobility, dementia care, and reporting changes.

Qualifications

• High School diploma or GED required.
• Experience in a skilled, sub-acute, assisted living or memory care setting preferred.
• CNA licensure if required by state.

Responsibilities

• Take and record vital signs.
• Measure height, weight, and fluid intake/output.
• Assist with medication reminders, bathing, grooming, dressing, escort service, and activities of daily living.
• Transfer, position, and turn residents.
• Contribute to the resident’s assessment and care plan.
• Care for residents with dementia.
• Attend required training, in-service, and staff meetings.
