Responsibilities

• Taking and recording vital signs.
• Measuring and recording height and weight, intake and output.
• Caring for the resident's environment.
• Assisting with bathing, grooming, and toileting.
• Assisting with eating and hydration.
• Observing residents' skin and reporting changes to the licensed nurse; providing skin care as needed.
• Meeting residents' mental health and social service needs.
