Responsibilities

• Completes assessment for assigned patient reflecting age-specific variables.
• Assesses patient/family educational needs, abilities, learning preferences, and readiness to learn.
• Anticipates problems related to disease, adjustment, complications, discharge needs, and family support.
• Develops and updates nursing plan of care with input from interdisciplinary team and patient/family.
• Initiates and coordinates communication with physicians and other healthcare providers.
• Assesses patient response to treatment and initiates appropriate nursing interventions.
• Prioritizes care needs and readjusts priorities with changing patient status.
• Participates in discharge planning from admission with patient teaching and referrals.
• Applies Standards of Care and follows OSHA policies.
• Includes patient/family as active participants in care planning and teaching.
• Administers prescribed medications and educates patient/family about medications.
• Provides pain management and comfort to patient/family.
• Coordinates care with hospital departments and community agencies.
