Qualifications

• Valid RN Compact State License.
• 3+ years RN experience in Emergency/Acute Care.
• Experience with clinical data collection and patient advocacy.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Bachelor's or master's in healthcare admin/Public Health is a plus.

Responsibilities

• Connect mobile resources to improve patient outcomes.
• Perform telephonic patient assessments using evidence-based protocols.
• Educate patients and promote healthy behaviors across the care continuum.
• Use telemonitoring tools to track progress and intervene early.
• Collaborate on care planning and mentor teammates.
• Represent GMR in community outreach and promote programs.
