Qualifications

• Valid RN Compact State License.
• 3+ years RN experience in Emergency or Acute Care.
• Experience with clinical data collection and patient advocacy.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Bachelor’s or Master’s degree in healthcare administration or public health is a plus.

Responsibilities

• Be a catalyst for change by connecting mobile healthcare resources and coordinating services.
• Telephonic patient assessments using evidence-based protocols.
• Educate patients on care options across the healthcare continuum.
• Leverage telemonitoring tools to track progress and intervene as needed.
• Collaborate in team care planning and mentor colleagues.
• Represent GMR’s programs in the community and outreach efforts.
