Responsibilities

• Conduct comprehensive patient assessments across medical, psychological, developmental, sociocultural, spiritual, and lifestyle factors.
• Develop and implement individualized care plans; evaluate and revise regularly.
• Administer medications and treatments per physician orders and protocols; educate patients and families.
• Coordinate with medical staff and departments to optimize patient care and communication.
• Document patient information, care provided, and outcomes accurately in the medical record.
• Evaluate care plan effectiveness and adjust for positive health outcomes; ensure infection control and safety compliance.
• Provide emotional support and therapeutic communication to patients and families.
• Supervise and delegate tasks to healthcare team members.
