Qualifications

• RN licensure in good standing
• Associate degree in Nursing required; bachelor's preferred
• 1+ acute care hospital experience preferred
• Proficient in patient education and coordination of care
• Strong communication and teamwork skills

Responsibilities

• Conduct comprehensive patient assessments across multiple domains
• Develop and implement individualized care plans and adjust as needed
• Administer medications and treatments per orders and protocols
• Educate patients and families about conditions, treatments, and self-care
• Collaborate with medical staff to coordinate high-quality patient care
• Maintain accurate documentation of care activities and outcomes
• Monitor and improve the effectiveness of care plans
• Ensure infection control and safety standards are followed
• Provide emotional support and therapeutic communication
• Supervise and delegate care within the team
• Engage in ongoing professional development and performance improvement
• Respond to code situations and stabilize patients
