Job summary

Communities of Care is seeking a Home Care Nurse (LPN/RN) to join our nursing team in patients' homes in Minnesota. This full-time role supports a 2-year-old client with a trach, vent, and tube feeds, with a potential 7pm-7am schedule and flexible nights. New grads welcomed and training provided to ensure compassionate, quality care.

We are hiring a Home Care Nurse (LPN/RN) to join our nursing team to support our clients in their homes. We offer world class home care with small community values. We are looking for Full-time Registered Nurse or Licensed Practical Nurse work with a 2 year-old female in her home. We welcome new grads and will train!

Details for this client's case:

• 2 year old with history of prematurity.
• Has trach, vented, tube feedings, and quite active!
• Proposed schedule:
• NOC: 7:00 PM - 7:00 AM - flexible
• Client lives in Hoyte Lakes with her family.

Duties:

• Administer medications and treatments as prescribed by the physician

Requirements
