Job summary

Communities of Care seeks a dedicated Home Care Nurse (RN or LPN) to support a 2-year-old client at home. This full-time position in Minnesota welcomes new grads and provides training.

You will administer medications, monitor vital signs, assist with personal care, and collaborate with healthcare professionals to implement care plans. NOC shift 7:00 PM–7:00 AM, flexible schedule.

We are hiring a Home Care Nurse (LPN/RN) to join our nursing team to support our clients in their homes. We offer world class home care with small community values. We are looking for Full-time Registered Nurse or Licensed Practical Nurse work with a 2 year-old female in her home. We welcome new grads and will train!

• 2 year old with history of prematurity.
• Has trach, vented, tube feedings, and quite active!
• Proposed schedule:
• NOC: 7:00 PM - 7:00 AM - flexible
• Client lives in Hoyte Lakes with her family.

Duties:
